General Booking Forms:

• Reservation Instructions- for reservations aboard all vessels.
• Reservation Form - to be filled out and faxed or mailed in order to confirm a reservation made with that office.
• Booking Terms - Information on required deposits, cancellation terms, change fees, etc.
• Application Form & Liability Release Form - To be filled out and sent to your booking office to confirm your reservation. A copy of the Application/Liability Release Form MUST be given to the crew upon boarding the vessel.

• Overall Dive & Trip Insurance information - Explorer Ventures strongly encourage all guests to secure insurance prior to diving and traveling. Dive accident insurance is required for the Galapagos & Maldives.
• Medical Form (237kb) - Recreational Scuba Training Council
• Nitrox Waiver (16 kb) - required to be signed on board prior to diving with nitrox. Copies are available on board as well; we do not need to receive this form in the booking office prior to your trip
